How it is meant to work
PD-1 is the brake on an activated T cell. Oncology antibodies release that brake so the immune system attacks a tumour; rosnilimab presses it, to quiet the T cells attacking the follicle. Monthly subcutaneous dosing was supported by the healthy-volunteer phase 1.
Why it matters
The mechanism has a striking piece of corroborating evidence behind it: PD-1 blockade in cancer patients is a recognised cause of alopecia areata. A drug doing the reverse is a rare case of a hair-loss hypothesis whose human experiment has already been run, in the other direction.
